What is the PHQ-9?

The Patient Health Questionnaire-9 (PHQ-9) is a clinically validated tool used to screen for depression and measure the severity of depressive symptoms. It's widely used by healthcare professionals to help assess a person's mental health.

Instructions:

Below are 9 questions about your mood, interest in activities, and overall mental well-being over the last two weeks. Answer each question based on how you've been feeling. Once you complete the questionnaire, you will receive a score and some guidance on the results.

Scoring and Interpretation:

  • 0–4: Minimal or no depression

  • 5–9: Mild depression

  • 10–14: Moderate depression

  • 15–19: Moderately severe depression

  • 20–27: Severe depression

If your score is 10 or above, we recommend reaching out to a mental health professional for further evaluation.
